A method for rehabilitating the patients with the functional disorders and consequences of the focal lesions of central and peripheral nervous system comprises the continuous rehabilitation post-hospital treatment at home providing the conditions of hospital facility. Prior to rehabilitation post-hospital treatment, the physical, somatic, neurological and psychological state of the patient is examined for the delineation of the area aimed at the earliest restoration of the pathologically altered function. The clinical facilities in home setting are provided according to the conclusion of the expert appraisal elaborating the rehabilitation route for the individual patient containing the steps of the continuous rehabilitation treatment and the control terms for the dynamic assessment of the efficacy of the said rehabilitation treatment. When the control terms are attained, the additional laboratory and instrumental electrophysiological and neurovisualization monitoring of the state of the patient is performed.Спосіб реабілітації хворих з функціональними розладами та наслідками вогнищевого ураження центральної і периферичної нервової системи включає неперервне реабілітаційне лікування на післягоспітальному етапі в умовах місця проживання хворого шляхом створення стаціонару на дому. Перед початком проведення неперервного реабілітаційного лікування попередньо виконують експертну оцінку фізичного, соматичного, неврологічного, психологічного стану хворого та визначають зону найближчого відновлення патологічно зміненої функції, після чого за результатами вказаної експертної оцінки створюють стаціонар на дому шляхом визначення і встановлення технічних засобів відповідно до стану хворого та розробляють реабілітаційний маршрут хворого, в якому визначають етапи здійснення неперервного реабілітаційного лікування та контрольні строки для динамічної експертної оцінки ефективності проведення вказаного реабілітаційного лікування, причому при досягненні зазначених контрольних строків
