A trajectory guide for (102) for introducing an instrument into a human or animal subject is described. A base (104) can be configured to be affixed in a burr hole in a human or animal subject. The base can include a socket (120) configured to be located in the burr hole when the base is affixed to the subject. A spherical ball (106) can be configured to be located in the socket and to be pivotable with respect thereto. The spherical ball can be located in the burr hole when the base is affixed to the subject. The spherical ball can include therethrough a pivotably adjustable instrument guide ball passage (107) providing an adjustable instrument trajectory defined longitudinally by the ball passage. An engageable ball retainer (108), can be configured to be located within the burr hole when engagably coupled to the base that is within the burr hole. The retainer can be configured to secure the ball to inhibit pivoting of the ball to hold the instrument trajectory constant. An elongate guide stem (110) of the guide can comprise a distal end configured to engageably concentrically align its lumen with the ball passage, such that the guide stem extends longitudinally outward from and is supported by the ball. The guide stem can be configured to be removed in sections without disturbing the aligned instrument. An access port portion of the trajectory guide can be left in place, without disturbing trajectory alignment, and can allow overlying skin to be sutured closed. The access port can provide infusate delivery, such as using an injection port, catheter or the like.
