PROBLEM TO BE SOLVED: To implant an improved intervertebral disk safely and efficiently.SOLUTION: A template 154 for marking score lines on a spinal segment having a superior vertebral body, an inferior vertebral body, and an intervertebral disk includes a shaft 160 and a template marker 164 provided at a distal end 156 of the shaft and having a cruciform-shaped structure. The cruciform-shaped structure includes a first vertical arm 166 and a second vertical arm 168 extending away from the first arm and aligned with the first vertical arm along a first axis.
