The present invention includes an image forming device housed within a mobile device housing and configured to enable multiple image acquisition modes. An apparatus according to the present invention includes an image sensor, a first optical assembly, a second optical assembly, and a control portion. A first optical assembly of the present invention is positioned over a first image acquisition area of an image forming surface to define an image for any object plane in a first clock on a first image acquisition area and defines a first clock. The second optical assembly is positioned over the second image capture area of the image forming surface to define a second clock to form an image for any object plane in the second clock on the second image capture area.
