Disclosed are an apparatus and a method for determining a similarity of a lesion of a medical image in the present invention. According to an aspect of the present invention, the apparatus for determining a similarity of a lesion of a medical image comprises: an image input portion for receiving a reference image including a reference lesion area and a target image including target lesion areas； and a similarity determining portion for respectively applying an advantage weighting which is increased as being closer to the center of the reference lesion area to a pixel which is included in a first area among the target lesion areas and applying a penalty weighting which is increased as being further from the reference lesion area to a pixel which is included in a second area among the target lesion areas, and determining the similarity between the reference lesion area and the target lesion areas.의료 영상의 병변 유사도 판단 장치 및 방법이 개시된다. 일 양상에 따른 병변 유사도 판단 장치는, 기준 병변 영역을 포함하는 기준 영상 및 대상 병변 영역을 포함하는 대상 영상을 입력 받는 영상 입력부와, 대상 병변 영역 중 제1영역에 포함되는 픽셀에는 기준 병변 영역의 중심에 가까울수록 증가하는 어드벤티지 가중치를, 대상 병변 영역 중 제2영역에 포함되는 픽셀에는 기준 병변 영역에서 멀어질수록 증가하는 페널티 가중치를 각각 적용하여 기준 병변 영역과 대상 병변 영역의 유사도를 판단하는 유사도 판단부를 포함할 수 있다.
