A dose indicator comprises a base 14 for mounting to an upper end of a container 3 of a pressurised metered dose inhaler 1, an annular inner wheel 11, an annular outer wheel 12, a drive member 13, a transmission cog 17 and a flexible cover 16. The drive member, inner wheel and outer wheel are concentrically mounted on the base to rotate about a first axis. The transmission cog is mounted on the base between the inner and outer wheel to rotate about a second axis parallel to the first axis. The inner wheel comprises a plurality of primary indexing teeth on an inner annular surface and one or more secondary indexing teeth on an outer surface. The outer wheel comprising indexing teeth on an inner annular surface. The transmission cog comprises a plurality of gear teeth on an outer face which, in use, engage the indexing teeth of the outer wheel and also engage one or more secondary indexing teeth of the inner wheel. The drive member being mounted within the inner wheel and comprises a body having one or more drive arms which engage the primary indexing teeth of the inner wheel.
