A drug eluting balloon device (10,40) is disclosed, comprising a balloon (20,50,72) and a sheath (30,60,74,94) disposed around the balloon, the sheath comprising a plurality of drug delivery areas (32,62,78,95). When the sheath is in a first position, the drug delivery areas are in the form of pockets (34,64,78,95) adapted to contain a therapeutic agent. When the sheath is expanded, the pockets invert into protrusions in order to deliver the therapeutic agent (36,66,80,96). In an alternative embodiment, the pockets are adapted to be pushed outwardly upon inflation of the balloon.
