An arrangement for heart valve replacement comprises a prosthetic system (70), comprising a prosthetic valved body (72) and an annular support structure (60) within which the prosthetic valved body (72) can be expanded until it meets opposition, the annular support structure (60) having a locking mechanism (63) and a guide wire adapted to be deployed below the annulus of the native valve so as to provide an effective guide for the positioning of the annular support structure. Each component of the annular structure is made with a hollow geometry, allowing the passage of a guide wire (46), and by tensioning the guide wire (46) a closing action is applied to the locking mechanism (63).
