PROBLEM TO BE SOLVED: To fabricate a selection button that correctly functions even when a resin sheet material having excellent durability and waterproofness is used.SOLUTION: An information collection device comprises a first resin layer on which a communication module is mounted and a second resin layer adhered on the first resin layer. The first resin layer comprises a first surface having: a plurality of contact points two of which are provided associated with each position of a plurality of selection buttons a plurality of terminal parts connected to each of a plurality of terminals of the communication module and a plurality of conductive patterns which electrically connect each of the plurality of contact points with each of the plurality of terminal parts. The second resin layer comprises a second surface having a plurality of contact point conduction parts which are provided for corresponding two contact points.
