A system and method for providing a video game that maps macular acuity including multiple-choice exams in which a fixation point is ensured by simultaneous presentation of both central and perimeter targets. The game may be implemented on a hardware platform that includes a video display, a user input device, and a video camera. The camera is used to monitor the ambient light level and the distance between the device and the subject's eyes. The game functions as a macular vision perimeter that generates a map of eye vision that can be compared to normative data. The type of visual acuity to be examined is preferably vernier visual acuity, but resolved visual acuity can also be examined. Test results are transmitted to medical professionals by telecommunications means to facilitate diagnosis or monitoring of age-related macular degeneration or other related eye diseases.
