The template (1) has a base (2) having through openings (19-21) and sleeves (3-5). The first surface (9) of base has a footprint (10) of preparation site and a second opposing surface (15) of base has support surface (16-18), such that through openings are expanded starting from surface (15) to surface (9). The through openings and support surfaces are shaped, so that the sleeves are partially inserted into through openings. The sleeves are supported on support surfaces in an inserted state and are aligned relative to base.
