An injection device is provided with dual medicament chambers and a mixing pen needle assembly, whereby predetermined dosages of two medicaments can be provided to a mixing reservoir in the pen needle assembly and delivered simultaneously with the injection device. The pen needle assembly has a patient end needle, a filler needle, and the mixing reservoir intermediate and accessed by the patient end needle and the filler needle.
