Images of eye provide useful information for physical examination and health assessment for human. In the current method usually a direct ophthalmoscope or indirect ophthalmoscope or a slit lamp is used for examination of eye and taking images of fundus. The optical and electronic version of direct ophthalmoscope, indirect ophthalmoscope, slit lamp and fundus camera are available. Direct ophthalmoscope is light weight and can be carried easily. Other instruments are heavy and require considerable ser up. All these apparatus require the patient and health care professional to be in the same place. A remote examination and assessment is difficult with these types of apparatuses. Current apparatus does not have ability to provide an initial assessment of disease / disorder of the eyes The method and apparatus invented enhance the current method of physical examination, converge functionality of all these separate instruments and extend it to remote assessment by using a specific apparatus involving digital and communication technology. The apparatus consists of a specific type of optical lens with a specific LED illumination unit and a digital device with an monitoring unit. While performing the physical examination of eye, the light from the LED illumination unit is either placed on the outer surface of the eye of the patient or made to pass through dilated pupil of the patient. The (still or video) image of the outer surface of the eye or of the fundus including the retina is taken using the new apparatus, as the physician or the health care assessor requires it to be. The apparatus allows the physician or the healthcare assessor to view the patients body eye from near or remote place and to instruct how the apparatus would be placed. The apparatus captures the image of eye / fundus / retina body and transfers the body sound on real time to the physician or health care assessor from the remote place.
