A system for monitoring patients includes an automated computer based platform communicating with a plurality of monitoring devices. The monitoring devices are provided to a plurality of patients. The information from the monitoring devices is analyzed automatically and if parameters, such weight, glucose level, EKG, etc. are found to be abnormal, an alert is sent to the physician in charge of the respective patient.
