An adjustable intraocular lens (AIOL) for implantation in a capsular bag of a patient's eye comprises first and second components coupled together to define an internal fluid chamber and an external fluid reservoir. . The internal area of the AIOL provides optical power to one or more of the shaped fluid in the internal fluid chamber, or the shape of the first or second component. The fluid reservoir comprises a bellows region in which one or more folds of the bellows extend circumferentially around the optical axis of the eye. The bellows engages the capsular bag and the compatible folding region between the inner and outer bellows portions allows the AIOL profile to deflect when the eye is adjusted to near vision. As the eye is adjusted, fluid moves between the internal fluid chamber and the external fluid reservoir to provide an optical power change.
