The pathway (101) may include a proximal portion (102), a distal portion (106) and an intermediate portion (104) disposed between the proximal portion (102) and the distal portion (106). A reflex angle may be defined between channel (118) defined by the intermediate portion (104) and a trough configuration defined in the proximal portion (102).
