The present disclosure provides various embodiments of a sheath for a rotating intravascular probe for insertion into a blood vessel. An exemplary sheath includes a flexible portion having a lumen for receiving an ultrasound probe, a distal portion including a flexible multilayer end, and a transition portion connecting the proximal and distal portions. The multilayer end defines a guidewire lumen having a distal guidewire opening and a proximal guidewire opening through the sidewall.
