A pre-loaded stent graft delivery device (100) and stent graft (131) comprises a stent graft having at least one fenestration (147) or side arm, the fenestration being preloaded with an indwelling guide wire (138, 146). Indwelling access sheaths (118, 122) are provided within auxiliary lumens (92, 94) of a pusher catheter (112) and dilators are preloaded into the access sheaths (118, 122) along with the indwelling guide wire (138, 146). A handle assembly (130) at a distal end of a guide wire catheter (172) includes a multiport manifold (114) with access ports (202, 204) to the auxiliary lumens (92, 94) in the pusher catheter (112). Upon deployment of the stent graft (131) into the vasculature of a patient, the indwelling guide wire (138, 146) can be used to facilitate catheterization of a side branch or target vessel through the fenestration (147) or be used to stabilize the access sheath (118, 122) during catheterization, advancement of the access sheath (118, 122) into the target vessel and deployment of a covered or uncovered stent therein through the access sheath (118, 122).
