A respiratory treatment exchange having an exhalation conduit (104) with a valve (318) which controls the flow of gas between the interior of the conduit and the atmosphere. The valve can comprise a flexible or pivotal cover (2185) which blocks an aperture in the exhalation conduit. The cover is biased or tensioned towards a closed position such that a differential pressure is required to actuate the valve and allow air to pass through the aperture. The vent may be configured to require a positive pressure in the exhalation conduit before it actuates and vents the air to the atmosphere (HPVE side). The valve may further have a configuration (LPVE side) which allows air to flow from the atmosphere into the exhalation conduit when a sufficient negative pressure is formed in the exhalation conduit.
