Systems and methods for model-based coronary calcium (CAC) scoring are provided. In order to more easily identify coronary arteries and other related anatomical features in the image, a model image of the heart region is aligned with an image of the patient's heart region. When the images are aligned, relevant calcium plaques are identified based on their presence in the coronary arteries, and the relevant plaques are then labeled by the particular coronary artery in which the plaque is located. Coronary arteries with labeled plaques are individually scored based on plaque size and x-ray attenuation, and then based on all relevant plaques, the overall score associated with the patient's risk of coronary artery disease is Calculated.
