The therapeutic energy may be microwave energy or may be thermal energy derived from microwave energy. The apparatus includes a device having a spin tip portion and a fluid delivery mechanism for transporting fluid to and from the treatment site. The fluid delivery mechanism includes a rigid insertion element that is arranged to extend into the treatment site, where the fluid can be aspirated from the treatment site and a substance is injected into the treatment site to replace the aspired fluid.
