The present invention relates to a facial symmetry evaluation device and a method for manufacturing the same. A facial symmetry evaluation device (100) according to an embodiment of the present invention includes: a fixed wafer (110) where a mounting portion (120) is formed on one side surface, a maxillary tooth facing portion (111) is formed on the upper surface, and a mandibular tooth facing portion (112) is formed on the lower surface, wherein the maxillary tooth facing portion (111) has an engraving shape corresponding to a maxillary tooth shape and tooth arrangement, and the mandibular tooth facing portion (112) has an engraving shape corresponding to a mandibular tooth shape and tooth arrangement the mounting portion (120) formed on the side surface of the fixed wafer (110) for a symmetry evaluation extending portion (130) to be mounted and the symmetry evaluation extending portion (130) mounted on the mounting portion (120) and extending in a vertical direction of a face (10) along an imaginary horizontal symmetrical line (13) of the face (10).
