A method for manufacturing a drug core for sustained release of a therapeutic agent. The method comprises: (a) Injecting into a precursor sheath body, at temperature of less than about 25 degrees Celsius, a mixture comprising a therapeutic agent and a silicone matrix to substantially fill the precursor sheath, the precursor sheath being substantially impermeable to the therapeutic agent; (b) Curing the mixture, at a temperature greater than about 25 degrees Celsius, in the precursor sheath body to provide a cured, filled precursor sheath body containing a precursor drug core; (c) Dividing the precursor sheath body to form a plurality of drug inserts. The drug inserts can be inserted into an implant body adapted for disposition within or adjacent to a body cavity, tissue, duct or fluid of a patient, in particular the implant body is a punctual plug. The therapeutic agent can be glaucoma medication, a muscarinic agent, a beta-blocker, an alpha-agonist, a carbonic anhydrase inhibitor, a prostaglandin, an anti-inflammatory agent, an anti-infective agent or a dry eye medication; in particular the therapeutic agent can be latanoprost.
