An energy conveying coaxial structure for delivering RF and/or microwave energy to an electrosurgical instrument. Its core is an insulating layer 142 forming a lumen 130 of 1-5 mm diameter for passage of an electrosurgical instrument. An inner conductive layer 140 covers the outside of the core, followed by a dielectric layer 138 and an outer conductive layer 136. This structure may be an integral part of a flexible insertion tube (104) or a separate liner. The outer conductor and dielectric may be integral whilst the instrument channel, inner conductor and remaining dielectric form a removable liner. An overhanging stop flange 170 at the distal end of the insertion tube may limit distal movement of the instrument tip (112). Contacts 156, 158 on a connection collar 152 on the instrument couple to the conductive layers via terminals 160, 162. The structure is incorporated or inserted into an endoscope, laparoscope or the like which may have an observation channel 128 and be used for NOTES surgery.
