PROBLEM TO BE SOLVED: To accurately reproduce the position and direction of a socket installation shaft during a surgery as planned before the surgery.SOLUTION: A guide instrument for hip replacement arthroplasty includes: a columnar drill guide 11 provided with a through-hole 11A for a drill an arm holding part 12 holding a pair of arm parts 12L and 12R to be abutted on an acetabular cup edge part, which are provided on the distal end side of the drill guide 11, and for which an extension direction is turnable about an axis A12 orthogonal to the axial direction of the drill guide 11 and an extension position is adjustable a column 14 attached to the drill guide 11 and orthogonal to the axial direction a target guide 16 provided with a through-hole 16A for a guide pin for piercing an acetabular cup and abutted to the pelvis at the distal end and a slider 15 to be slid on the column 14, capable of holding the target guide 16 in parallel with the drill guide 11 and adjusting the axial direction position.
