Provided herein are methods for determining and implementing optimized dosing of therapeutic anti-CD47 formulations on a schedule that provides for safe step escalation of the dose while achieving therapeutic levels in a clinically effective period. The method may include the steps of cleaning, escalating, and maintaining. In one embodiment, the dosage regimen comprises administering an initial (i) sub-therapeutic dose of an anti-CD47 agent or (ii) a cell decay therapy to achieve safe levels of circulating tumor cells for subsequent treatment (cleaning) )； Elevating the dose of the anti-CD47 agent until the therapeutic dose is reached (stepwise expansion)； And maintaining (maintaining) a therapeutic dose for a period of time sufficient to reduce tumor cells in the patient's bone marrow.
