The lead (50) has a set of microcables (40 1-40 5, 40 7), where each microcable comprises an electrically conducting core cable (11) that is connected to a pole of a multipolar generator of an active implantable medical device. A polymer insulation layer surrounds the core cable, where the polymer insulation layer has an exposed area (30). The set of microcables is coupled together in a twisted configuration within the lead, where the microcables together form a multipolar detection /stimulation distal active portion of the lead. The distal active portion is equal to 0.5mm.
