PROBLEM TO BE SOLVED: To further improve the quality of a subtraction image while reducing an exposure dose for a subject in a radiation image processing device, method, and program for generating a subtraction image.SOLUTION: A first image acquisition part 31 acquires a first radiation image taken on a first image-taking condition, and a second image acquisition part 32 acquires a plurality of projection images taken by tomosynthesis imaging on a second image-taking condition. A reconfiguration part 34 reconfigures the plurality of projection images and generates a plurality of tomographic images in a tomographic plane of a subject. An image composite part 35 generates a second radiation image using the plurality of tomographic images.
