A head fixation device for use in a medical procedure has a pair of arms defining a skull clamp, wherein each of the pair of arms has an upright portion. The head fixation device further has a skull pin assembly connectable to the upright portion of a select one of the pair of arms defining the skull clamp. The skull pin assembly has one or more skull pins that selectively contact a patients head. The head fixation device further has an accessory attachment interface to receive a first accessory for use during the medical procedure. The accessory attachment interface is positioned along the upright portion of the select one of the pair of arms defining the skull clamp having the skull pin assembly. The accessory attachment interface is positioned substantially proximal to the skull pin assembly having the one or more skull pins that selectively contact the patients head.
