Devices and methods for identifying patients at high risk for developing decompensated heart failure (HF) are described. Devices and methods can stratify risk using sensor signals that are selected according to the chronic condition of the patient. The medical device receives input of a patient condition including at least a first chronic symptom indicator and a second chronic symptom indicator, senses one or more physiological signals from the patient, and the first chronic symptom. A plurality of signal metrics can be generated from the physiological signal if the indicator and the second chronic symptom indicator meet their respective criteria. One or more patient-specific signal metrics may be selected from the group comprising signal metrics that are selected for both the first chronic condition and the second chronic condition.
