The present disclosure describes a handpiece for transarterial delivery of a therapeutic substance to the inner ear. The handpiece can be inserted into the middle ear by a surgical tympanotomy approach. The handpiece may allow a therapeutic substance to be injected in a controlled manner directly through the round window membrane and into the inner ear. Direct delivery of the therapeutic substance into the inner ear may allow the delivery of an accurate amount of the therapeutic substance into the inner ear. Since the therapeutic substance is delivered directly to the inner ear, the delivery of the therapeutic substance is a molecular size constraint and inconsistent diffusion that exists when the therapeutic substance is diffused across the round window membrane.
