A method of deep brain stimulation for treating Alzheimer's Disease, Multiple Sclerosis, Minimally Conscious State and/or Mood Disorders, vascular brain disorders, where there is small vessel compromise, hypertension and/or epilepsy utilising dual frequency / dual target neuromodulation along a single lead traversing the medial thalamus or fornix into the lateral periaqueductal gray (lPAG) or ventrolateral periaqueductal gray (vlPAG), applying high and low frequencies, respectively. The lead may be routed via a burr hole and optional cap 57. An apparatus for such treatment comprising first 55 and second 54 electrode regions, preferably comprising a plurality of electrodes, split into multiple parts, with insulating materials between the parts. The centers of the electrode regions are separated 10-20mm along an electrode lead 52, preferably with insulating material between the regions. A controller applies pulsed electrical signals: a low frequency modulation to the first electrode region, at the distal end of the lead, and a high frequency modulation to the second electrode region, proximal to the controller.
