An electrocautery instrument for endoscopic therapy of the present invention includes an operating handle and a rotatable positioning means. The operating handle includes a cylindrical portion, a clamping portion, and a positioning protruding portion. The clamping portion and the positioning protruding portion are disposed on the cylindrical portion which further has a first inner hole. The rotatable positioning means encloses the cylindrical portion and includes a rotatable part, a second inner hole, an engagement part, and fixing units. The engagement part is disposed inside a side wall of the second inner hole and engages with the clamping portion. The rotatable part is provided for rotating the rotatable positioning means, so that the positioning protruding portion can be positioned at any corresponding fixing unit.
