A barbed suture including abody with barbs on the periphery, a pointedend, and an anchor at one end to resistmovement of the suture in the direction of thepointed end is provided. One or more limbson the anchor may be provided, which maybe arcuate, of varying lengths, and varyingdistribution about the periphery of the suturebody. Other anchor designs are provided.Methods of placing single-directional andbi-directional barbed sutures to approximatethe tissue on each side of a wound and toposition and support tissue in the absenceof a wound, as in cosmetic surgery, areprovided, and may include terminal J-stitchesor S-stitches. Methods of placement may bemade with sharp, pointed ends, which maybe needles, or insertion devices. Sinusoidalpatterns of sutures that have amplitudesgenerally perpendicular to the resultantholding force of the suture are provided.
