A system and method for generating amedical history that is determined based on the patient'smedication list. Medications are taken for specificindications, i.e., disease and/or symptom, and thesystem includes a knowledge base of all known medicationsand associated indications. Preferably, an expertsystem allows the patient, nurse or other user, to enterall or part of the name of the patient's medications, andcreates a list of probable medications by determiningwhich of the known medications the patient most likelytakes. Based on a user selected probable medication,the system creates a list of probable indicationsassociated with the selected medication. Based on a userselected probable indication, the system generates amedical history for the patient. The probable medicationsand probable indications are determined preferablybased on the patient's demographic data, historicaldata for other patients, and responses to follow-upquestions generated by the system.
