Methods and systems for treating chronic total occlusion, particularly CTOs that are difficult to treat, are disclosed. In this approach, re-communicating the CTO is accomplished using a combined forward and backward approach. The proximal end of the occlusion is pierced using a forward-directional wire, using conventional approach. Using side branch blood vessels, the distal end of the occlusion is traversed in a counter-repulsive manner.
