The invention relates to an actuator (1) for an aerosol inhalation device, comprising a housing configured to receive an aerosol container (2) containing a pressurized dosage form, a mouthpiece through which the user inhales, a nozzle block (5), a passage (8) and a tubular element (11) extending in the mouthpiece from the passage opening in a longitudinal axis aligned with the longitudinal axis of the mouthpiece. In particular, said tubular element is positioned so as to enclose a passage opening within a recess. The tubular element is designed in such a way that one of its end openings can fit snugly on the outer surface of the nozzle block around the passage opening so as to be on a continuous path of flow along the passage. Such a configuration provides for a significant reduction in the inhaled coarse fraction of the aerosol drug due to inertial collision and retention in the actuator rather than the oropharynx, followed by a decrease in the associated side effects and oral candidiasis in the patient.
