An access device (60) for percutaneously accessing a pedicle fastener (51) connected to a vertebra of a patient, the pedicle fastener (51) having a head (53) defining a receptacle for receiving a spinal fixation rod (5) therein, and the pedicle fastener (51) having at least one blade (52) detachably connected to the head (53), comprising:an elongate body (65) having a distal end for engagement with the head (53) of the pedicle fastener (51), the body (65) defining a longitudinal bore (78) therein along a central longitudinal axis of the body (65), characterized in thatthe body (65) includes a groove (79) therein extending longitudinally along the bore (78), the groove (79) dimensioned to receive the at least one blade (52) of the pedicle fastener (51) therein.
