A biopsy device includes a body, a needle, and a cutter actuation mechanism. The needle extends in a distal direction from the body. The cutter actuating mechanism rotates and translates the cutter about the body and against the needle. The cutter has an overmold that provides a lead screw and a hexagonal portion. The cutter actuating mechanism includes a first gear disposed around the hexagonal portion of the overmold and a second gear and the second gear is engaged with the lead screw of the overmold by a nut integral with the second gear. The cutter actuating mechanism is operable to simultaneously rotate the first gear relative to the body at a first rotational speed and the second gear relative to the body at a second rotational speed.
