The Advanced Chronic Disease Self-Management Program (ACDSMP) is a multi-disciplinary, science-based behavior modification program which seeks to empower the patient to manage his own chronic diseases and disorders such as hypertension, asthma, arthritis, obesity, diabetes, heart disease, lung disease and stroke recovery. The program is designed with a breakthrough approach that can help make healthy living become second nature. Healthy eating, walking and better breathing- and staying motivated to continue these - are the building blocks of the program. It involves a combination of eating plan, walking plan, breathing technique and other lifestyle modifications. ACDSMP is a patient centric pro-active approach that works to prevent progression of the chronic disease state and thereby reduce the occurrence of resulting health complications which require expensive acute intervention. Participants are better situated to manage their own conditions and remain healthy for a longer period of time, increasing the ability to maintain independence which will also accordingly decrease the need for long-term care in a residential facility, a great added expense which may be avoided or delayed. This easy-to-follow ACDSMP gives patients the tools they need to manage the challenges of living with a chronic health condition and will result in significant, measurable, and sustainable improvements in health status, self-efficacy, and psychological well-being, along with increasing exercise, reducing fatigue, and enhancing partnership with physicians.
