A pharmaceutical composition for sustained release for topical application to the skin surface, comprising: a pharmaceutically active agent comprising one or more steroids, from 0.01% to 5% by weight of the total weight of the pharmaceutical composition of a fatty acid ester, water, C2-C6 monoalcohol, - fatty acid, and - from 0.05% to 5% by weight of gelling agent, where the ratio of the mass of the fatty acid ester in the composition to the entire active agent in the specified composition is at least 4: 1, ether fatty acid ratio: active agent, p preferably 4: 1 to 20: 1.2. The composition according to claim 1, which further comprises a co-solvent, preferably propylene glycol. The composition according to claim 2, in which the co-solvent is present in an amount of from 0.01% to 7% by weight of the total weight of the pharmaceutical composition, preferably from 3% to 7% by weight. The composition of claim 1, wherein the fatty acid ester is selected from the group consisting of ethyl oleate, isopropyl oleate, isopropyl myristate, isopropyl isostearate, isopropyl palmitate, ethyl octanoate, ethyl dodecanoate, ethyl linoleate, ethyl palmitoleate, ethyl isostearate and ethyl linolene. The composition of claim 1, wherein the fatty acid ester is isopropyl myristate. The composition of claim 1, wherein the fatty acid ester is ethyl oleate. The composition of claim 1, wherein the fatty acid ester is an ester that can be formed by reacting the fatty acid included in the composition with an alcohol. The composition according to claim 1, in which the fatty acid ester is not an ester that can be formed by reacting the fatty acid that is part of the composition with an alcohol�1.
