PROBLEM TO BE SOLVED: To provide a medical surgical instrument wherein an electric knife and a laser knife can be switched, wherein irradiation of a laser beam is not disturbed by blood or body fluid of a surgical field, and wherein an optical fiber is not damaged.SOLUTION: A probe 5 of an electric knife-combined type laser knife 1 is set to be electroconductive, and the probe 5 is connected to a high frequency oscillator 39. An operation mode of the electric knife can be switched by hand switches 13A-13C. An optical fiber cable 7 is introduced in the probe 5, and the laser beam can be radiated to the tip of the probe 5 from a tip part 7c of a core wire 7b of the optical fiber.
