PROBLEM TO BE SOLVED: To provide an apparatus capable of preserving and displaying information so as to be confirmable optionally by setting and recording the date and the time zone when a medicine is taken latest.SOLUTION: A character rotary sheet 12 and a surface sheet 10 are superposed each other, and the central axis of the character rotary sheet is connected to the surface sheet, to thereby obtain a structure in which the character rotary sheet can be rotated around the central axis. Further, such a structure that a character or the like can be read from a character display window 20 of the surface sheet is formed, and a present date and a time zone when a medicine is taken are recorded by rotating the character rotary sheet, to thereby enable confirmation of a medication record at any time.
