The present invention relates to a novel use of gemigliptin as an agent for treating diabetes. The pharmaceutical composition comprising gemigliptin, an active metabolite thereof or a pharmaceutically acceptable salt thereof as an active ingredient for preventing or treating vascular complications has an antidiabetic effect derived from inhibition of DPP-4, an antioxidative effect, an AMPK activation effect, an effect of inhibiting advanced glycation endproducts (ACE) production and crosslinking of AGE with proteins, and an effect of reducing proteinuria and protecting kidney in Type 1 and Type 2 diabetes models. In addition, the pharmaceutical composition effectively inhibits production of albumin urine, damages on renal tubules, fibrosis of renal tissues, an increase in collagen expression in renal tissues, an increase in expression of fibronectin and phosphorylation of Smad3 protein. Thus, it is shown that the gemigliptin has an effect of treating diabetic nephropathy, cardiomyopathy, neuropathy and retinopathy, an effect of inhibiting vascular neointima formation and an effect of reducing vascular calcification. Therefore, the gemigliptin can be used as an agent for preventing and treating vascular complications.COPYRIGHT KIPO 2016본 발명은 당뇨병 치료제인 제미글립틴(Gemigliptin)의 신규 용도에 관한 것으로, 본 발명에 따른 제미글립틴(gemigliptin), 이의 활성 대사체 또는 이들의 이들의 약제학적으로 허용되는 염을 활성 성분으로 포함하는 것을 특징으로 하는 혈관 합병증의 예방 또는 치료용 약제학적 조성물은 DPP-4 억제에 의한 혈당 강하 작용 이외에도, 항산화 작용, AMPK 활성화 작용, 최종당화산물(AGE) 생성 및 AGE와 단백질의 교차결합에 대한 억제 효과가 있고, 제1형 및 제2형 당뇨병성 모델에서 단백뇨 감소 및 신장 보호 효과가 있으며, 당뇨병성 신증 환자에서 발견되는 알부민뇨의 생성, 세뇨관 손상, 신조직 섬유화, 신조직의 콜라겐 발현 증가, 신조직의 피브로넥틴 발현 증가 및 Smad3 단백질의 인산화를 효과적으로 억제할 수 있으므로, 당뇨병성 신증, 심근병증, 신경병증 및 망막병증 치료 효과와 함께, 혈관 신내막 형성 억제 및 혈관 석회화 감소 효과가 있다는 것이 확인되므로 혈관 합병증 치료 및 예방용 약제로 사용될 수 있다.
