The spinal implant comprises an outer sleeve (2), an inner core (3) and a nut (4) all made of a biological inert metal alloy and centered on a common axis. The nut engages the external screw threads of the core and has an externally accessible array of bevel gear teeth such that rotation of nut causes axial displacement of the core relative to the sleeve.
