A system is disclosed for use with a clot located in a target vessel for managing one or more acute ischemic events. The system can include a guidewire extending in a longitudinal direction from a proximal end to a distal end, the distal end being configured to control its orientation relative to the clot and the target vessel. The guidewire can include one or more sensors disposed on or adjacent the distal end, the one or more sensors configured for sensing properties of the clotin vivoand treating the clot based on the sensed properties.
