Devices and methods for the treatment of chronic total occlusions are provided. One disclosed embodiment comprises a system and method of facilitating treatment via a vascular wall defining a vascular lumen containing an occlusion therein. The system and method includes inserting an intramural crossing device into the vascular lumen, positioning at least the distal tip of the crossing device in the vascular wall, advancing an orienting device over the crossing device such that an orienting element of the orienting device resides in the vascular wall, inserting a re-entry device, and re-entering the true vascular lumen.Linvention porte sur des dispositifs et des procédés de traitement docclusions chroniques totales. Dans une exécution, il sagit dun système et dun procédé effectuant le traitement via la paroi vasculaire délimitant la lumière vasculaire siège de locclusion. Le système et le procédé consistent: à introduire dans la lumière vasculaire un dispositif transversal intramural, à placer au moins lextrémité distale du dispositif transversal dans la paroi vasculaire à avancer un dispositif dorientation sur le dispositif transversal de manière à ce que lune de ses parties se place dans la paroi vasculaire et à introduire un dispositif de repénétration qui pénètre dans la véritable lumière vasculaire.
