Compositions with improved particle transport in mucus are provided. In some embodiments, the compositions and methods include preparing mucus-penetrating particles (MPP) without any polymer carrier, or with minimal use of the polymer carrier. The composition may include modifying the surface coating of particles formed of a drug having low water solubility.
