An arthroscope (1) comprises a sleeve (20) having an internally hollow, proximal portion (21 ) suitable to be at least partially positioned in contact with the body of a patient and an internally hollow middle portion (22) connected to the proximal portion (21) so as to create a through axial cavity. The sleeve (20) has, within the middle portion (22), an abutment area (27). The arthroscope also comprises an optical unit (10) having an elongated portion (11) and a coupling portion (12) suitable to be at least partially received inside the through axial cavity of the sleeve (20). The coupling portion (12) has a matching area (17) suitable to couple with the abutment area (27). The matching area (17) and the abutment area (27) are shaped so as to have at least one pilot element (17.1, 27.1) so as to determine a univocal predefined coupling position between the sleeve (20) and the optical unit (10)； the matching area (17) and the abutment area (27) are also shaped so as to guide and automatically place the optical unit (10) in such a predefined position.
