Screw holes 40a, 40b, 40c, 40d screwed to the skull around the barhole are provided on the auxiliary circle 54 in the vicinity of the outer periphery of the intermediate portion 36. In the vicinity of the protruding ends of the two protrusions 32 a, 32 b, a screw hole screwed to the skull is provided outside the auxiliary circle 54.
